About LiveWell Chronic Disease Management
Program Overview
The Saskatoon Health Region's LiveWell Chronic Disease Management Program is offered to individuals and their families who have a chronic disease such as bleeding disorders, diabetes, chronic obstructive pulmonary disease (COPD), heart disease, sleep apnea, asthma, arthritis and other chronic conditions. The LiveWell CDM Program is aligned with the strategic direction of Transforming the Care and Service Experience and Partnering for Improved Health for Aboriginal people.
The CDM Team
An inter-professional team of medical specialists, nurse clinicians, dietitians, pharmacists, exercise/physical therapists, social workers, peer leaders and support staff provide individual and group education/counselling, exercise and specialty clinics.
Program Options
Different program and service options are available to assist clients and families as they adapt and adjust to their chronic disease. Education sessions, supervised exercise programs, individual counselling, disease management clinics, self-management and stress management workshops provide a variety of options for adopting healthy lifestyle choices.
CDM Program Goals
- To develop and implement coordinated, effective and efficient care for people with chronic conditions.
- To optimize care of people by promoting a team approach and enhanced self-management of disease.
- To promote inter-professional collaboration and education.
