Chronic Disease Management
St. Paul’s Hospital leads Chronic Disease Management in the Saskatoon area in partnership with the Saskatchewan Health Authority.
Our interdisciplinary team of experts in Chronic Disease Management assists clients and families as they adjust to living with chronic disease. Programs and services across the region include education group sessions, supervised exercise programs, individual clinical counselling, disease management clinics and self-management workshops.
Chronic Disease Management also works in partnership with other important groups and programs to spread messages of health and well-being throughout the community.
Location
The Chronic Kidney Disease Program at St. Paul's Hospital is located in B Wing on the third floor.
