The Kidney Health and Education Centre will consolidate kidney health services into one central location on the second floor of St. Paul’s Hospital. This consolidation will simplify access to information and support for patients and their families, while improving processes for doctors, nurses, and support staff. Additionally, this will create the space required to establish the new Transition Program.

The Transition Program provides intensive support and education tailored to the individual needs of each patient. The program empowers patients by giving them the skills and knowledge needed to manage their kidney treatments effectively. Patients graduate with the confidence to continue their care in their communities or even in their own homes. This approach reduces patients’ reliance on hospital care, leading to cost savings and improved patient outcomes.
